My name is Elisa Bergquist, and I am the owner of Hometown Health and Wellness in La Porte, IN. I am a family nurse practitioner with 20 years of healthcare experience. I started as a nursing assistant and continued my education to become an RN and then a nurse practitioner. I have worked in family practice, school based health, and long term care as a nurse practitioner, but my passion for holistic health and wellness drove me to open my own direct primary care office. When not working, I enjoy spending time with my husband and family doing outdoor activities such as hiking, swimming, kayaking, gardening, and traveling.
